TITLE OF POSITION: Nurse Aide-1 (NA-1)
REPORTS TO: Agency Director and Registered Nurse(RN)

JOB SUMMARY:
The Nurse Aide-1 is an unlicensed paraprofessional member of the health care team who works
directly under the supervision of a registered nurse (RN) and performs various services for the
client, the majority of which are personal care activities of daily living (ADLs). Activities of
Daily living include assisting the client with mobility, eating, bathing, dressing and
toileting/continence management. The Nurse Aide-I is assigned to assist in a manner that will
promote a quality, safe environment for the client.
DUTIES AND RESPONSIBILITIES:

• Performs only under the supervision of a RN who has provided written

instructions for plan of care (POC)

• Performs total care or assists client in all activities of daily

living/personal care, such as, but not limited to, bathing, dressing,
personal hygiene and incontinence care

• Assists clients with transfers, ambulation, and exercises as assigned.
• Prepares nutritious meals within the client's diet and assists clients with

eating within the care setting.

• Performs light housekeeping chores, which facilitates client self-care in

the home care setting.

• Meet the safety needs of the client and use equipment safely, properly

(e.g. bed rails, assistive devices.

• Takes and records accurate vital signs (temperature, pulse, respiration,

blood pressure) and reports any changes to the RN when assigned.

• Adhere to all Agency policies.
