Position Summary:

  • Provide quality direct nursing services following the resident service plan in an environment that promotes residents’ rights, dignity, freedom of choice, and person-centered care.
  • Essential Responsibilities:
  • Provide treatment and care following the resident service plan.
  • Provides Quality Assurance.
  • Acting Supervisor when DON and Director of Maple Woods are out of office.
  • Administer medications skillfully and correctly on a timely basis.
  • Implement, evaluate and provide nursing services as directed.
  • Observe and record signs, symptoms, behaviors, reactions of residents.
  • Support and ensure compliance with established infection control procedures, universal safety precautions, and state/federal regulations.
  • Coordinates the work activities of the Resident Assistants that are appropriate with skill level. Assists with staffing coverage of shifts. Provide input into the evaluation of staff performance with recommendations for appropriate action.
  • Display concern and initiative while maintaining an acceptable standard of resident care.
  • Identify problems/concerns with resident or staff and help to guide to solutions.
  • Work collaboratively with employees to create a climate that promotes engagement and provides opportunities for growth, learning and job satisfaction.
  • Accurately and promptly implements physician’s orders.
  • Order medications from pharmacy and upon their arrival, distributes them to the appropriate medication carts.
  • Maintain the standards of accurate and complete recording and reporting.
  • Communicates with physician’s offices, home health agencies, and families regarding health conditions/issues.
  • Schedules laboratory blood draws according to physician’s orders and follows-up with physician’s office when reports come back.
  • Assists with move-ins and discharge of residents.
  • Is resourceful and calm in emergent situations.
  • Make report to oncoming staff regarding resident information, needs and changes in condition.
  • Maintain a professional approach with confidentiality.
  • Understand and support Resthaven policies and procedures.
  • Completes Care Companions yearly performance evaluations.
  • Assumes all other position related responsibilities as assigned.

Knowledge, Skills & Abilities:

  • Effective written and verbal communication skills.
  • Computer literate and familiarity with Word, Excel and Email.
  • Demonstrated clinical competencies.
  • Communicates effectively and tactfully, while recognizing age, cultural orientation, needs, abilities and physical condition, while using dementia specific interventions when appropriate.
  • Ability to display a sense of caring, concern, compassion and dignity for residents.
  • Ability to establish and maintain effective and professional working relationships with residents, families, visitors, and employees.
  • Well organized and attentive to detail.
  • Ability to work independently and manage time productively.
  • Ability to operate standard office equipment including fax, calculator, telephone, copier, etc.
  • Reading, writing, grammar, and mathematics skills.

Education, Training, Experience:

1. Graduations from an accredited Licensed Practical Nursing or Registered Nurse Program.

2. Successful completion of the National Association LPN or RN state boards.

3. Previous nursing experience preferred, especially with the elderly population.

4. Meet on-going requirements to maintain current nursing license. Assumes responsibility for own continued personal and professional growth.

5. Attends all required in-service training and departmental meetings.

Job Types: Full-time, PRN

Pay: From $57,000.00 per year
