Work somewhere that cares! We are looking for Occupational Therapists to work on our Queens, NY Home care team.
Job Duties:
- Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy.
- identifies patient needs for other disciplines and services.
- Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
- Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
- Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients.
Requirement:
- 2 years of experience as an occupational therapist
- Licensure: Current registration to practice Occupational Therapy in New York State
