Job description
Position Summary:
The Registered Nurse (RN) Behavioral Health Care Manager is part of the behavioral health care team and is responsible for ensuring that care coordination and care management servicesare effectively and efficiently provided/available to patients within the primary/behavioral health practice who are most at risk for health deterioration, sentinel events,cost impact and/or poor outcomes in collaboration withthe applicable providers and clinical support staff.
The RN Behavioral Health Care Manager is responsible for ensuring that the primary care provider (PCP), behavioral health provider, and practice team effectively maintains a central role in coordinating and managing the care of these vulnerable patients so that the patients receive optimal care including behavioral health care, acute illness management, chronic diseasemanagement, and preventive care across multiple healthcare settings and multiple physicians/providers.
Position Requirements:
1. Registered Nurse with license recognized by the State of Iowa Board of Nursing and BLS certified.
