Social Services Coordinator - (TEMPORARY)

The Social Services Coordinator is responsible for managing the Health Care Center admissions process and for establishing and maintaining an open, effective communication between the community and referral sources, physicians, and third-party payers. This includes direct contact with medical and business professionals in the surrounding area to generate referral activity and soliciting direct referrals from these sources.

PRINCIPLE DUTIES:

  • Completes all mandatory documentation in a timely (determined) fashion as established by applicable regulations.
  • Maintains regular and on-going relationship with family to discuss needs or concerns, mediates issues that may arise between resident/family/staff.
  • Develops a comprehensive social history and assessment of the resident that includes problems and strengths, special needs, and preferences (social, religious and/or cultural) and the implications for the care plan.
  • Participates in the development of the individual’s care plan including the goals and approaches for the Social Services component.
  • Coordinates discharges/transfers to ensure the appropriateness and continuity of care. Develops a comprehensive discharge plan in coordination with residents/families and multidisciplinary staff.
  • Ensures that any Advanced Directives are complete and maintained and placed in resident charts.
  • Provide/identify referral agency and/or community resources to meet the needs of the resident/family.
  • Obtains referrals from hospital social workers, physicians, or family members keeping wait lists current.
  • Conducts admission interviews with the resident and/or responsible party, obtaining all required financial data, signatures, pertinent social information, and medical information, attending physician information, and other pertinent and required documentation prior to admission.
  • Reviews all admission related paperwork with the resident and/or responsible part including Medicare Part A coverage and non-coverage procedures. Maintains all non-medical record related paperwork in an admissions file for each resident.
  • Performs meet and greets and room orientation to include the presentation of a Welcome Gift.
  • Maintains the Inquiry Log daily to ensure completion and accuracy.
  • Informs the Medical Records/Administrative Assistant of on-going daily census activities such as admissions, discharges, deaths, room transfers, changes in payment methods, and temporary and permanent apartment status.
  • Keeps an ongoing daily/monthly census of residents in the Health Care Center noting admissions, discharges, deaths, transfers, changes in payment methods, and temporary and permanent assignments. Coordinates utilization review for new admissions covered by Medicare Part A.

QUALIFICATIONS:

  • Minimum of one year of LTC experience
  • Good working knowledge of Medicare Part A, certification procedures, and medical terminology.
  • Able to work flexible hours.
  • Familiar with federal and state regulations and requirements for LTC.
  • Must possess excellent written, oral, and interpersonal skills, as well as the ability to juggle multiple priorities.
  • Case management experiences a plus.
  • Must possess reliable transportation.

PHYSICAL REQUIREMENTS/WORKING ENVIRONMENT:

Normally works in a well-lighted, comfortable office setting. Must be able to work efficiently with frequent interruptions. Must be able to move freely throughout the facility and able to lift (unassisted) 50 lbs. May be subjected to offensive odors and exposure to chronically ill and frail elderly. Must be free from communicable disease.
