Courtyard Manor of Wixom is currently looking for a motivated, energetic, and caring limited or fully licensed Bachelor Level Social Worker to provide case management and psychoeducational groups & services for a caseload of 25-30 adult residents diagnosed with severe and persistent mental illness, co-occurring substance use disorders, and medical co-morbidities. Courtyard Manor of Wixom is licensed as an Adult Foster Care home serving 80 older adults across four buildings. The Social Worker/Case Manager provides services that include assessments, person centered planning, psychoeducational groups, individual case management services, crisis intervention, and discharge planning.
Primary Job Responsibilities include:
1. Completes Annual Assessments within the scope of practice for an LBSW on assigned residents.
2. Completes comprehensive monthly case management notes for all assigned residents evaluating monthly progress.
3. Assists residents in the identification of personal goals for their Person Centered Plan (PCP).
4. Develops treatment plans/Individual Plans of Service (IPOS) for residents upon admission and annually.
5. Completes quarterly reviews of the IPOS in a timely manner and discusses resident progress toward goals.
6. Participates in treatment team meetings with the multidisciplinary team.
7. Possesses skills or the ability to learn to lead self-help and psychoeducational groups with residents, including AA/NA, Nicotine Anonymous, and Schizophrenia Alliance groups.
8. Provides individual case management services for assigned residents as listed in the resident’s IPOS.
9. Provides crisis intervention/supportive services as needed
10. Facilitates the development of incentive reward and/or behavior plans as needed; monitors progress, documents progress in the IPOS; and communicates with CMH case manager.
11. Possesses knowledge and skills to complete discharge summaries and discharge instructions.
12. Completes accurate billing in the EMR as instructed.
13. Completes group notes within 24 hours of self-help or psychoeducational groups.
14. Facilitates visits with guardians, CMH case managers, and others as assigned.
15. Participates in clinical staff meetings.
16. Follows time-frame requirements for all required documentation in the record of all persons served.
17. Maintains confidential and secure records for all persons served.
18. Other related duties as assigned.
Qualifications/Specific Requirements:
Bachelor’s Degree in Social Work. Must possess an LLBSW or LBSW in the State of Michigan. Behavioral Health and/or Community Mental Health experience strongly preferred. Experience working with an Electronic Health Record and Microsoft Office (Word and Excel) preferred.
Physical Requirements/Working Conditions:
To fulfill job requirements, the employee must be able to work in a fast-paced environment with frequent interruptions. Must possess excellent time management or organization skills. Must be able to perform physical tasks of writing/computer use for extended periods of time; facilitate multiple groups per day; and work independently with minimal supervision.
Work schedule: Monday – Friday, 8:30-5:00 pm. No nights, weekends, or on-call.
